What is Money Follows the Person?

Money Follows the Person (MFP) is a federally funded program that helps NC Medicaid-eligible North Carolinians living in inpatient facilities move into their own homes and communities with supports.

The North Carolina MFP program:

  • Helps people who live in places like hospitals, nursing homes, or a care center for people with disabilities move into homes in
    their own communities.
  • Gives more choice for where people want to live and get help.
  • Looks for ways to fix problems that make it hard for people to get care at home.

Are You Eligible?

To move with MFP, a person must:

  • Live in a hospital, nursing home, or a care center for people with disabilities for at least 60 days 

  • Be eligible for community-based Medicaid

  • Approved for home and community-based services

  • Choose to move into a “qualified home,” like:

    • Their own home or apartment
    • A family member’s home
    • A group home with 4 or fewer people (only allowed in some cases in NC)

What Help Can You Get?

  • Accessing Medicaid Support:  People in the MFP program can get community services faster. This includes special Medicaid programs.

  • Planning Your Move Home: The program offers help with moving.
    Help includes paying for:

    • Staff training
    • Rent deposits
    • Starting utilities like electricity
    • Furniture
    • Home changes like ramps
    • Other one-time things, like food
